Application Forms
If you or a loved one are in need of urgent financial support, download and print the Financial Assistance Application form. Once completed, seal the form in an envelope addressed to Charis at NSBC and deliver it to the church office. Alternatively, you can hand deliver the sealed envelope to any of the Charis Steering Committee members. See the Contact Us tab for the names of the members.
Choose one of the following options:
- Request Urgent Financial Assistance - if there is a situation where a member or regular attendee of NSBC is in need of financial assistance from the Charis ministry
- Request Charis Outreach Project Funds - if you are requesting funds from the Charis ministry for a potential outreach project where NSBC volunteers will be reaching out to hurting people in our local community

All efforts will be made to keep confidential the names of the beneficiary and the related circumstances driving the need. Please be aware that supporting documentation may be requested in order to substantiate the request for financial assistance. Wherever possible, financial assistance payments will be made directly from the Charis care ministry directly to the creditor(s).
